Evaluation forms are now online, accessible from your 2014 IMFS account. Evaluations are required for every session that you wish to earn continuing education credits. Continuing education credits will not be issued until each evaluation has been completed in full.

Please follow the below instructions to complete the evaluation form after each session you attend.

ONLINE EVALUATION FORM INSTRUCTIONS

1. Make sure that you have your customer ID and password for your 2014 IMFS account. Your customer ID is the number that is on your name badge beneath the bar code. If you do not have your password, use the right side of the form of the login page to recover your password.
2. Log in at the 2014 IMFS Login Page
3. Scroll down until you see the Track Evaluation buttons.
4. You will see a button next to each track workshop, plenary, intensive, and luncheons (if registered). You must complete each evaluation for which you expect to receive credit. Every question must be answered unless otherwise noted.
